RN Case Manager, Home Health

The employer is seeking a dedicated Registered Nurse (RN) Case Manager to join their home health team. In this role, you will be responsible for the overall management of patient care plans, conducting initial assessments upon admission, and identifying appropriate nursing diagnoses. You will evaluate medications for potential adverse reactions and establish measurable goals consistent with each patient's specific diagnosis and needs. Typical duties for this position include informing physicians of clinical interventions, implementing nursing care within the scope of the Nurse Practice Act, and maintaining accurate documentation. You will be expected to re-evaluate patient needs regularly and ensure all progress and discharge notes are submitted in a timely fashion. This role requires strong communication skills to keep medical personnel informed of changes in patient conditions.
